May be availed of at Forticare clinics or any designated diagnostics and laboratory procedure after six (6) months from the date of effectivity of the contract.
- Taking of medical history
- Medical examination
- Chest X-ray (PA)
- Complete blood sugar count (CBC)
- Fasting blood sugar (FBS)
- Urinalysis and fecalysis

- Complex and non-complex diagnostic and laboratory examinations or procedures shall be covered up to the Annual Benefits Limits (ABL)
- Assistance in administrative requirements through Forticare Liaison Officers.
Emergency Care Services
Emergency care services
May be availed of in emergency as defined on the contract agreement & coverage after the enrollment on the health care program.
- The member, directly or through his representative should inform FORTICARE within twenty-four (24) hours from the start of the emergency;
- Up to Php25,000 per availment of ambulance service from a Non-Accredited to an Accredited Hospital;
- Member or any of his representatives should inform FORTICARE within twenty-four (24) hours. However, in case the member, due to his medical condition, is unable to communicate directly or through his representatives, the 24 hours notifications period can be extended for twenty-four (24) hours from the time he is clinically able to do.
Special Laboratory
Special laboratory and diagnostic procedures – In-patient procedures, may be availed at forticare clinics and accredited providers.
- X-ray
- Computed tomography scan (CT scan)
- Ultrasound
- 2D echocardiography with Doppler
- Magnetic resonance imaging (MRI)
- Electromyography (EMG)
- Nuclear test
- Other tests or procedures deemed appropriate by Forticare
