May be availed of at Forticare clinics or any designated diagnostics and laboratory procedure after six (6) months from the date of effectivity of the contract.
- Taking of medical history
- Medical examination
- Chest X-ray (PA)
- Complete blood sugar count (CBC)
- Fasting blood sugar (FBS)
- Urinalysis and fecalysis
Preventive Health Care Services
Preventive health care services may be availed at forticare clinics and accredited providers.
- Periodic monitoring of health concerns.
- Consultation on wellness
- Counseling on family planning
- Immunization excluding the cost of the drug/vaccine
- Inclusion in Forticare sponsored health seminars
Out-patient Health Care Services
Out-patient health care services may be availed at forticare clinics and accredited providers.
- Up to fourteen (14) out-patient consultations each year
- Up to ten (10) of pre-natal and post-natal consultation
- Referral to our accredited medical specialist
- Treatment of minor injuries
- Surgery that does not require hospital confinement including cataract extraction except the cost of intraocular lens
- Eye, ear, nose, and throat treatment
- Special diagnostic and laboratory procedures when medically indicated and required by an accredited physician. (ex. CT scan, MRI, or other tests deemed appropriate by Forticare

Hospitalization and In-patient health care services
- Complex and non-complex diagnostic and laboratory examinations or procedures shall be covered up to the Annual Benefits Limits (ABL)
- Assistance in administrative requirements through Forticare Liaison Officers.
Dental Care
Dental care coverage
- Up to six (6) Consultation per year
- Up to two (2) Oral Prophylaxis, each year starting on the fourth (4th) month upon issuance of the health care policy
- Tooth extraction, except surgery for the impacted tooth
- Treatment of minor mouth lesions, wound,s and burns
- Temporary filling and re-cementation of loose jackets, crowns, inlays, and onlays
- Routine gum treatment
Special Laboratory
Special laboratory and diagnostic procedures may be availed at forticare clinics and accredited providers.
- X-ray
- Computed tomography scan (CT scan)
- Ultrasound
- 2D echocardiography with Doppler
- Magnetic resonance imaging (MRI)
- Electromyography (EMG)
- Nuclear test
- Other tests or procedures deemed appropriate by Forticare
