Dental Healthcare Services, may be availed of at Forticare Clinics or any accredited dental clinics and shall include:
· Up to six (6) Consultations per year;
· Up to two (2) Oral Prophylaxis each year starting on the fourth (4th) month upon issuance of the health care policy;
· Tooth extraction, except surgery for impacted tooth;
· Temporary filling and re-cementation of loose jackets, crowns, in-lays and on-lays;
· Treatment of minor mouth lesions, wounds and burns; &
· Routine gum treatment
